TPS63000: TPS6300 keeps getting burned

Hello, I have a design where two TPS63000 are supplied by lab power supply at 5V or a battery at 2,4V,. One of them is set to provide 5V, other is set to provide 3.3V.

While my PCBs were under test, 3 of these PCBs have TPS63000 burned, Looking at the datasheet, these components are protected from short circuit currents. At the input I don't see any possiblity of over voltage. However at the output, there are several possibile reasons which can cause disturbances, for example I have connectors for microcontroller programmers etc. 

Using multimeter, I detect a short circuit at the input pins and not at the output pins. I would be grateful if you can share some insight that can help me debug my design. For example what are possible situations at the output that can damage these components.

           is it  burned when powered with 5V lab power supply?  

           i think it is possible voltage of Vin PIN is overange of the ABS value during switching. voltage of PIN Vin is not only input volatge, but also some ripple and spikes. The longer cable between lab power supply and PCBS, ripple will be larger.

           If i am right, please make the cable as short as possible and twisted pair cable is very helpful.

  • Yes, It is supplied by lab 5V supply. Cable connections are good. I measured the spikes at the input using oscilloscope, maxiumum I recorded is 5.6V

    I have several boards with these regulators. Only 3 have these regolators burned at the input side. I am interested in understanding the possibility of damaging these components from any distrubance at the output pins. Because I have 2 microcontroller programmers that use the output from these regulators as thier reference voltage.

       Do you means the spikes or ripple of output pins?  that is ok and  can not  lead to the damage of chips .

       BTW, 5.6V is oscilloscope captured?
