3.It has nothing to do with the schematic diagram. The customer has not been on the computer yet and PCB has not been involved, but the electrical performance parameters are tested and found that the standard voltage is not in conformity with the specification.

    The devices that have been flagged look to show signs of damage. It is common for the quiescent current of damaged devices to increase, and the quiescent current in all of the the flagged devices has increased by nearly an order of magnitude. All TI parts are tested for functionality before being shipped, so it seems likely that the parts have been mishandled or have been exposed to conditions that may exceed the abs max ratings of the devices before being tested, such as large ESD voltage. There also exists the possibility of reverse current which can damage the device if the test setup is not carefully designed to avoid the condition VOUT > VIN + 0.3V.
