TPS25990: Startup issue on the TPS25990 design.

Hi Team, 

We are facing a startup issue in our TPS25990-based design.

Instead of 12V in the E-Fuse output, we are getting only 2V. 

We have probed the waveforms. Attached are the waveforms, schematics, and TI E-Fuse calculation sheet for reference. 

Note:

The waveform captures SWEN and VOUT signals, it is understood that the design is going under some fault and the E-Fuse keeps retrying.

Similarly, PGOOD signal is low and FAULT signal is pulled high. 

Kindly provide debugging suggestions on this issue as soon as possible.
