TPS61236P: Powering Up Chip Fail

We are experiencing a very infrequent chip failure that has occurred twice over the past 8 months of 40 boards with several thousand power cycles.

The TPS61236P is powered (V_SYS) from a Li+ battery source/charger.  SYSTEM_ENABLE is activated by inserting a data connector.

Below is a schematic.

Can see any issues with our implementation?

    I can't tell the reason to failure by a schematic. Do you have the failure waveform and what kind of failure are you experiencing. Does the failure device fails to start up or the device can work but behaves unnormal?  On what test conditions does the devices occur failure

    About the schematic, I would like to know the inductance of L1, other designs are fine.
