Please refer my comments below.

RT resistor value is slightly off from 22.1kOhm .
Check C294, C295. Please check if the ESR is enough large to dampen a resosance caused by L9 / a long input cable.
Consider increasing the inductance value close to 10uH.
Connect VCCDIS to ground if not used.
Please 12V_VDD node to the output of the buck converter..
Assuming the purpose of R288 is to slow down the switching speed, it should be placed between the HB pin and the positive connection of C382)
