LM3485 power supply circuit fails

Hi,

I have a circuit with an LM3485MM that has failed 5 times on me out of about 15 circuits I've built.  It fails after hours of good operation.  When it fails, it puts the input rail of 21V-28V on my 5V line.  Is there something in the circuit I got wrong?

I've used this same circuit using a 12-15V power supply without anything like this happening.  I don't think its a quality control issue.
