LM3410 not working

LM3410 is not working. I'm getting steady Vin (which is 5V) on pins 1, 4 and 5 and 0V on 2 and 3. It is not switching. It is frustrating because it should be very simple but it is just now working.

Right now I'm using an external source to supply 15-16V to LCD that is operating at 100mA although it can go up to 300mA. I'm just mentioning this to prove that LCD is working alright and over-voltage/open circuit protection may not be the case.
