PTD08A020W High Temperature Fault

I have a new design with several PTD08A020W and PTD08D210W modules controlled by a UCD9248 controller.  One of the PTD08A020W modules has a high temperature fault with a temperature reading of 112 as reported through the TI Fusion Digital Power Designer software.  The module next to it has a temperature reading of 30.  There is a series sense resistor on the output of each module and I have removed those resistors while I debug these circuits.  So there is no load or current output from the failing device.  With the resistors removed I still  see a high temp fault.  I have tried clearing the fault and logged fault but the fault still persists.   We have used a thermal I have seen this on two different boards same module on both boards. Analysis with a thermal camera yielding no hot spots on either board. 

 

Have you seen this type of fault before?  Can you recommend a course of action?
