TPS54560-Q1: High failure rate about TPS54560-Q1 output

My customer use TPS54560-Q1 in their power board. About one-sixth of the defective rate of trial-produced products. The Vin is 12V and the designed output is 5V, but the output is 12V in failure board.

Can you share the layout for this design? Also under what conditions does the damage occur? Is this during start up/steady state?

Second is this a TPS54560B or TPS54560 non-B orderable part number?
