TPS65720: Device fails to shutdown DC/DC1 switching output when HOLD_DCDC1 pulled LOW, AC input pin low, and all configuration registers set to default values

Part Number: TPS65720

As the title says, in my design the TPS65720 fails to shutdown DC/DC1 switching output when HOLD_DCDC1 is pulled LOW, AC input pin is low / unconnected, and all configuration registers set to default values.

This seem very similar to the issue reported here where I don't see a resolution posted: https://e2e.ti.com/support/power-management/f/196/t/862559?tisearch=e2e-sitesearch&keymatch=TPS65720. I have an oscilliscope capture and schematic snippet that I can share if that would be helpful. This problem has presented itself on both prototypes that I've tested but I cannot replicate this issue on a TPS65720EVM board. Do you have any suggestions of what I can try next?
