LP3943: Output pins shorted to ground (EOS?)

I have a product that is using the LP3943 as a GPIO expander (per the app note).  The LP3943 is being used to load a counter in a CPLD.  On several devices we have seen shorts appear on some of the LP3943 output pins to ground.  We sent the faulty components to a local lab to be decapped and inspected.  The lab could not see any signs of EOS although they did conform that the shorts remained via a curve tracer.

Has this come up before concerning this part? Would a TI apps engineer be able to review the design with us to determine if we are perhaps not using the part in accordance with TI's expectations?

    LP3943 can be used as the IO expander, we didn't find any concerns of this device. Would you mind to submit the FA process with contacting TI local quality team to find the root cause and send me schematic to review?
