CC2650MODA: SmartRF Flash programmer doesn't recognize custom board with cC2650MODA

Hi,

I'm trying to bring up a custom board with the CC2650MODA using the SmartRF Programmer 2 v1.8.2 and a CC2650 LaunchPad. 

I get the error message: "The supported device could not be identified when I connect the USB cable to the LaunchPad.

I can see that nRESET is high, TCK toggles, TMS toggles.

Note: I am able to program the CC2650MODA BoosterPack with this same setup.  I've checked that my circuit connections match those on the BoosterPack.

Here is a comparison of my schematic and the BoosterPack schematic:

I tried to do a "force mass erase" and get these console messages:

>Initiate access to target: XDS-L1003317 using 4-pin cJTAG.

>Create XBAL object failed: Debug interface is locked.

>XBAL EB pointer is null.

>Failed to create device object.

Here are scope plots of nRST, TCK, TMS, TDO at the time when the USB connection is made to the LaunchPad with BoosterPack and then custom boards connected.  The falling edge of TMS was used as the trigger.

First with the BoosterPack connected:

Any debugging suggestions greatly appreciated!
