An op amp really isn't the function to accomplish the waveform you propose. Modern applications take advantage of Arbitrary Waveform Generation technques to generate complex waveforms such as yours. It is in its simplistic description is a digital to analog converter (DAC) that is driven by a clocked, look-up table containing a list of code that represents the desired waveform. The output of the DAC can be conditioned to provide the output voltage range that is needed.
