TLV2556: Mixed signal read on ADC

Hi,

I am using TLV2556 ADC for reading the voltage supply readings in my design. In circuit we have analog and digital supplies with different grounds. ADC is supplied from Analog supply and analog ground.

In ADC channels I am reading mixed supplies some are digital(digital ground) and some are analog(analog ground). I am facing problem in ADC data because ADC is using analog ground as a reference so it is not able to read digital signals(reference to digital ground).

 I have connect both the grounds with ferrite bead at one place in circuit but that is far from this ADC. May be this long loop for digital ground could be the issue. Please provide solution to solve this issue.

Following is the circuit pictures.

Is it mandatory to have jumper connection(ferrite bead or zero ohm resistor) between AGND reference of this ADC and Digital ground plane in board near to this ADC?
