TPS25750: Missing table for decoding ADCIN1/ADCIN2 pins

My colleagues and I have thrashed this subject about for days... how to decode the ADCIN pins to determine operational mode and I2C address.  After some Googling, I encountered a couple of E2E threads where a TI super-user/employee (Chuck Branch) posted a picture of "Table 8-2. Decoding of ADCIN1 and ADCIN2 Pins", which clearly defines what the previously-vague numbers 0 through 7 mean.  But I cannot find that table in either of the two PDFs I have related to the TPS25750 (the datasheet itself and the "Host Interface Technical Reference Manual").  Multiple questions by the original poster to Mr. Branch about the location of the table went unanswered.  See here for the original thread:

So, what documentation is this elusive Table 8-2 listed in so I can add it to our digital library for future reference?
