in the datsheet there are some applications related to how use the i2c level translator.
in the picture the typical star application from datasheet.
Is it possible to do the star application after the level translation on B side or do mixed star topologies?

The B-side pins are special and must not be connected to any other special pins.
The number of devices does not have anything to do with this; you can connect multiple devices anywhere.

Multiple TCA9517 devices can be connected in any configuration as long as the "special pins" that Clemens pointed out do not connect together. I.e. B-sides cannot be connected together. TCA9517 A-side to A-side connection is allowed, A-side to B-side, but B-side to B-side connections are prohibited.

Is there only 1 TCA9517 connected in this circuit?
This should work as long as I2C-n do not represent TCA9517's with B-side connect to another B-side.

Hi Tyler, yes, there is onlu a TCA9517 in this circuit. The b side is connected to a several standard I2C devices.

This implementation looks like it would work as long as all 4 I2C-n traces can support the static voltage offset of ~0.52V.
I am assuming all I2C-n lanes use 3.3V logic which means the VIL = 30% of 3.3V = 0.99V, which the TCA9517 buffered LOW (0.52V) would support.
