TUSB8041: "unknown usb device " by windows 7

Hi,

I am in the board bring up process of TUSB8041 USB hub. I am using micro bo connectors for upstream and down stream ports. I am using no EEprom configuration or the default settings of the device. But when I am connecting to the computer I am getting "unknown usb device " error on windows 7.

Vdd33: 3.3V

Vdd: 1.2V (instead of nominal 1.1V)

Crystal: http://www.digikey.com/product-detail/en/445I23D24M00000/CTX1173CT-ND/3135114?WT.srch=1 

I checked the power and voltage is coming in. and all the pins looks connected normal. I am attaching schematic for further reference. Andy help is appreciated! 

is this some thing to do with GRSTz? what does GRSTz assertion mean? 

FYI:

For power on the device I tried connecting 1uF capacitor to the pin 50. Also, I tried using a power on reset device shown below which is supposed to be reset at 140ms:
