Input protection for SN65HVS883

I am using SN65HVS883 to detect the digital input signal for industrial . I had seen the datasheet and found that it has ESD protection and require a MELF resistor and capacitor. Normally, My input signal is 10 to 30 V and field supply voltage is 12V. My question are following:

1. Do I need more external protection for input such as overvoltage, surge protection?

2. Can I apply the input voltage more than field supply voltage?

3. If I don't want to place the LED at REx pin. How can I do to this pin?

    Our data sheets recommend external protection which would address concerns 1 and 2. I have included a screen shot below:

    As for concern number 3:

    Is there a particular reason why you do not wish to have LED's present?

    Thank you for answering.
    Refer to concern number 3, I need not to show the input status using LED and this can reduce some power consumption. I have seen in digital input stage, the LED is connected to the current limiter in series. But I can use the LED if it is needed.
