MSP432E401Y: USB Device Descriptor Request Failed.

I have a custom board with an MSP432e401Y. when attempting to connect the Device to a USB port on a PC, the device enumerates as an unknown device, because the Device Descriptor request failed. 

We've had a few hardware errors along the way, so we have reset the hardware following using the dbgjtag tool. At this point, I am trying to determine if the error is hardware, or firmware based. There are 3 boards we are using. Launch Pad Dev Board 1, Launch Pad Dev Board 2, and the Custom board. 

Launch Pad Dev Board 1, and the custom board have had to have the hardware reset. Launch Pad Dev Board 2 has not had the hardware reset. When running our custom firmware, LaunchPad 1, and the Dev Board return the same error when making a USB Connection. Launch Pad 2, which has not had it's dev board reset, enumerates as expected. 

This seems to indicate that there is some level of setup of the USB device that is erased when resetting performing the factory reset of the MSP432 device. Does this sound accurate, and if so, can someone point me to to a reference to set up USB enumeration?

Alternatively is this possibly a hardware issue that causes a failure in USB enumeration? Schematic is as follows, except that there is no 100Ohm resistor between USB pin 4, and MCU pin 95:
