LM5007: LM5007 power unstable issue

Hi Sir:

We have used LM5007 for Vin 24-48V Vout 12V/0.2A this application is ok, but we have else project Vin 16-24V Vout 12V/0.2A this case we production 8 pcs but just only 1 pcs can work.

Fail status is when we power on is work but pass few seconds output is unstable then shut down, then IC fail.

The attached file is the circuit difference, please help check.

    Hi

    the ripple injection to FB need higer than 25mV,  if there is no Cff in you schematic, than the ripple in FB will be less than 25mV, pls put 4.7nF or 10nF C30 see any improvement
