LM25118: My board putting out 1.35 Volts wen it should be ~12 Volts.

I have an other design that I based this off of, so I know that the components that I have will work. I originally made a design similar the the one generated by webench:

The input is 10-24 V max current is 7 Amps

After I verified that it works, I made a smaller version of the board so I could combine it with another board. The new board won't work. When first turning on, the output voltage is 11.8, but once a load is applied, the voltage starts to drop linearly with a bigger load. After turning it off, and back on, the voltage doesn't go back to normal. I have tried it with 2 different chips. I am not sure if I burnt them out, but they get hot. There are 2 sheets I combined onto one board, but the second is irrelevant as far as I know.
