TPS61196: The output strings randomly turn on

Hi Team,

Our customer is using TPS61196PWPR to control 4 strings of WLEDs (15 series LED) with series voltage of 46.5V typical @ 80mA/string. Everytime he turns on the power supply, the strings of LED turn on randomly, sometimes one string turns on sometimes two or three but not all four. 

He measures 43.3V total voltage from anode to cathode of the 15 LEDs. The PWM source can provide 5mA only to the PWM1 PWM2, PWM3 and PWM4 pins. 

He also added that the PWM input is always present since it is being generated from external IC - CDM10VXTSA1, with +3V level. The frequency should be like 1kHz (1.02Khz), see the attached picture. After a certain level of the external trimmer that regulates the PWM duty, the input signal become pure +3V3 DC. 

What could be the cause of this issue? Please see attached schematic diagram for reference.
