LM20154 NC pin not connected to GND

We are seeing failures of this part where the PVIN shorts to gnd and the part smokes.  The NC pin (5) in the design is floating.  Could this cause failure?

    Datasheet is rather emphatic about the connect to GND:

    5 NC These pins must be connected to GND to ensure proper operation.

    Figure 35 in the datasheet shows the potential recirculating currents, so I would say it is quite possible NOT having pin 5 connected could result in failure, assuming all other design criteria is within the specifications.
