From the schematic, you have protection at the RJ45 and a TVS at the PD. Have you tried placing a TVS between RTN (cathode) and VSS (annode) for additional protection of the IC?

Where is the ESD being directed at? At the RJ45 chassis? At one of the input leads? Is this air discharge? Where is the return connection of the ESD connected to? Earth ground?

If ESD is zapped at the RJ45, the first line of defense is the back to back TVSs, then the bob smith terminations, the emi filter should also add some impedance to steer the energy through the bob smith. Any other energy will be clamped by the TVS near the IC. If the additional TVS recommended above does not help, can we look at the layout?

Are you seeing any arcing during the ESD test?
