LM63615-Q1: Failure with LM63615-Q1

Hi,

I used LM63615-Q1 in my design at 5VDC output, the input voltage was 12VDC and the max load current drawn was 100mA. But unfortunately I had to see multiple failures almost 5 times out of 6.

Please suggest a reason/root cause analysis for this.
