DRV411 is used for a few years ,but recently a direct output of 5V happened when power on. what is the probable cause ?

Typically an error condition in the DRV411 can cause the output to hit the rail upon powering up. This can be found if either the ERROR pin or the OR pin are pulled low. You may have to connect a pull-up resistor to these pins to check the signal levels. Some possible error cases are:

Also, does problem occur with just one module, or several modules? Could you share the schematics with us?

Thank you very much! Add some information as below before further test:
1、The schematic is following Figure 56 in DRV411 datasheet and check by TI, which is used in the past 4 years;
2、The output hit the rail even though without input (Ip in Figure 56);
3、300pcs of 8000 modules do not work well, and more than 70% in these 300pcs occur this problem;
4、Replace a falling device with a new one resolve this problem;
5、The PCB has X-ray detection to exclude soldering problem;

If you have followed the debug steps I mentioned in the previous post, and believe the problem is with the DRV411 ICs, then you may initiate a return process for the failing ICs for a quick analysis of the issue. The process is defined at the link below:

If you purchased the DRV411 parts through a distributor, TI requires that you work through the distributor in order to return the material to TI. If you have more technical issues, then I will be glad to help you.
