LM358A: Different batches lead to failure

Dear Expert

We found in customer side different batches lead to failure .The failure rate is 100% !Could u give us some adivice about these ?

↓Use such marking TI part 100% failed at final test.  body info: “1C5X ”, Marking:”M6P”) , checked  with supplier such marking part made in China.

↓Replaced by Such marking TI part  from another source body info: “9C TI  ”, Marking:”M6P, tried 2unit both are passed the test.
