IF the TLV2316 is powered with a 0 to 5V supply ( V+ = 5V and V- = 0V (GND) ) and the input (+IN A) is also connected to ground (0V) and -IN A is tied to OUT A (unity gain configuration), what would be the output offset on OUT A from the negative rail (GND)? I am observing OUT A is equal to 0V with no offset from the negative rail.

The above described scenario is measuring the offset voltage while having the common mode at V- (in this case GND).
Figure 2 in the datasheet shows that the results you are seeing are typical. I have highlighted the VCM = V- scenario.
Note that 9 typical devices are shown and can have many different Vos depending on the unit (within typical spec).
