OPA4991-Q1: Using as a voltage buffer/follower design question.

We are using OPA4991-Q1 as a voltage buffer/follower. During the test, we found an issue but don't no why.

Please find addached PDF. The first page is our circuit of OPA4991-Q1 and second page is measurement result and caculation.

According to the result, when input voltage above 20V(OPA4991 input above 1.74V), the output value of OPA4991 will getting worse.

In other words. When OPA4991 input above 1.74V, the higher input voltage the larger distortion be found on the output of OPA4991.
