Getting Wrong output Voltage 

Please find the schematic, We are using LM358AD  and giving 5V to opam IN+ pins, but we are getting 1.7V TO 2.5.V on output pins change the frequency and the output voltage not content

    The LMV358A and LM358A are different devices; only the former is optimized for low-voltage applications. The LM358A does not have rail-to-rail I/Os; it will not work with input voltages above 3 V. (The LMV358A has a limited common-mode voltage range, too, but the worst that happens is that the electrical characteristics are no longer met.)

    Is LMV358A correct op amp? Please provide waveforms showing incorrect behavior.
