PGA204/PGA205 gain less than one

Hi! I assembled PGA204 and PGA205 on my board. It worked properly for few minutes and then crashed. More exactly the amplification gain is less than one now. Then I tried two new ones and assembled them according to the configuration recommended in the datasheet. 204 amplifies the input signal to maximum 4 times larger than the inital value. I have a feeling that I am treating them in the wrong way and expose them to damage unconciously, especially knowing that they are highly sensitive electrostatic discharge.

I appreciate if one can share the experiences with me and/or give me some instruction.

    Can you share your schematic with us so we can view your circuit connections?  Do you believe that you're accidentally applying ESD or EOS damages to the PGA204?  You may benefit from reviewing the content in the ESD and EOS TI Precision Labs videos: 

    Without knowing anything else about your system is it possible that you're trying to operate the output outside of the allowed VCM vs Vout range as shown in the figure copied below?  The graph shows the maximum allowable output voltage based on the common-mode voltage applied to the devices inputs.  For example, with a VCM voltage of +10V, the maximum output swing is only +/-5V.
