A properly functioning OPA548 should go into output disable mode when the E/S pin is connected to V-. I do not find anything in your schematic that would explain why that is not happening.
Do you have the heatsink that the OPA548 package tab is attached to floating (not connected to any potential), or connected to V-? If the package tab is grounded instead of floated or connected to V-, that would result in improper operation and possibly device damage.
