What is the output load it only shows the 1µF capacitor this may be to large of a capacitive load. What else is connected. Also how does the IN- Channel look? Is it similar to IN+.

What is the load connected when seeing the noise. All I see is the 1µF capacitor. In the simulation the load I am refereeing to is the load of the output of the INA186. The 1µF cap is at the output of the INA186 and I think it may be too large. That is why I asked what else the INA186 is loaded by.

We met serious issue now.
INA186 is burn out in many PCB, ADC also burn out which behind INA186.
INA186 VCM=-0.2 to +40V, PVDD=24V, is it safe condition?
Do you have idea what possible cause burn out issue?

Hello Ian,
It looks like your supply voltage is what spiked. Look at the Vref pin and it spikes really high and it is divided from the 3.3V ADC supply . Looking for the root cause of this could find you the root problems. The inputs (IN+ and IN-) can go upto 40V.
