Hi Sam,
This seems like an IC level issue. It seems like the 948 IC might be getting damaged. You need to send it to the quality team for ATE testing to see if there are any open/short continuity failures etc.
Please go to ti.com then do the following:
click on customer returns under quality & reliability
Click on CPR (Customer product return)
Fill out the form and submit your request then someone from quality team will be assigned to own this investigation.
