The pull-up voltages do not need to be the same as Vref_A or Vref_B; the only limitation is that no voltage must be lower than Vref_A. (This is an error in that figure.)
The LSF does not have digital inputs or outputs, so there are no VIx/VOx specifications. For voltages below Vref_A, it behaves like an analog switch. (When the switch is closed, it directly connects the A and B sides; its resistance is specified by ron.)

Yes this image is incorrect, I believe its supposed to say 5 V. These training videos on the LSF family will cover how these devices function and should help with implementation.
