Tool/software:

we converted 4 wire to 2 wire JTEG and also reduced the TCK frequency also wrapped the wires, but still i am facing the JTEG disconnection issue.

Not sure about the history on this issue. Is this your custom board ? Have you already referred to this appnote to follow the steps to debug such issues ?
