INA199: IC damage due to nanosecond over-voltage condition

I have redesigned my schematic to check the high side of the switch but I'm still seeing the output stuck at 0.03V. I'm looking at the inputs of the voltage pins and the voltage is rippling above 26V for ~20ns when the switches open and close. I'm assuming that is damaging the IC's?

If the 24V bench top supply is rippling above 26V on that timescale, what are my options?

Hey Michael,

This may seem like a bad question, but this new schematic is testing a new, unused INA199?

Ok now with that out of the way here are my other questions:

1. Was this INA199 ever functional before behaving abnormally?

2. Does the device ever cease this behavior and perform normally after a power cycle?

20 ns is a short time frame. More voltage probe data would be helpful as well as layout images. You can send these to my private messages if you wish.
