SLew > 300V/uSec
PS +/-10V or higher
noise 8uV/rthz (some latitude here)
unity gain stable
>80MHz BW at G=1
Pkg Dual SOIC (some latitude)

Is it necessary to be FET input?
If not, the THS4012 meets the above criteria. It is unity gain stable with a BW of 290 MHz, 310 V/us slew rate, 7.5 nV/rtHz noise, and comes in a dual SOIC package.
